This PDF form is used to officially request that a product be tested by emBoot Labs for its compliance to the PXE specification and correct implementation of its UNDI transport layer.

Your company must also sign and return the emBoot Labs PXE Certification Testing Agreement, a legal agreement that contains the terms and conditions for emBoot Labs’ product testing program. emBoot Labs will not be able to process any product testing applications from your company until the agreement is in force.

Along with this application, fifty (50) percent of the testing fee shall be submitted. This application will not be processed until emBoot Labs receives the required fee payment.

After the application and supporting documents are received and processed, an emBoot Labs staff member will contact you to discuss testing arrangements.
